This program allows your loved ones to select the environment in which they would like to live. It’s important that elderly individuals as well as people with disabilities have the right to be in control of their own lives. This service provides case management, support, advocacy, encouragement, community interaction, and problem solving to assure health, safety and well-being of participants.
